Cristina GROHOLSCHI, the contact person at the Herblay-sur-Seine dental centre, presents the collaboration with BAYARD SANTÉ and the project led by the teams. The ambition is to gradually develop the centre into a multidisciplinary space combining dental care, general medicine and specialist consultations.
CUBESANTÉ assisted teleconsultation contributes to this approach: a healthcare professional supports the patient on site, in contact with a doctor remotely. Alongside the centre, CUBESANTÉ@HOME and NURSECUBE nurses extend this approach into the home, particularly for people who have difficulty travelling.
Coordination with CHARLÈNE aims to connect patients, professionals and support providers. The goal is to facilitate continuity across the different places where care is delivered.
